To apply for participation in the 2011 Care Card program, please complete the Care Card Participation Agreement below.  This agreement must be submitted to The Board of Visitors office by June 10, 2011, in order to accomodate printing deadlines.

Please download the 2011 Retail Participation Agreement to your computer, fill out the appropriate fields, save the file, and then email it to: carecard@boardofvisitors.org
